THE SIGNAL
On June 1, 2026, Joint Commission issued the review process guide for its Responsible Use of Artificial Intelligence in Health Care certification. Reviewers trace at least two AI tools across the lifecycle, and the first tracer option is a high-risk clinical tool, "for example, deterioration prediction/early warning clinical decision support."
The tracer questions go straight to decision ownership. Reviewers ask "which implementation decisions required governance approval (for example, go-live criteria, threshold settings, user eligibility, documentation requirements)?" They ask "How are governance decisions regarding this health AI tool or system being tracked (for example, minutes, approval memo, intake record)?" They also ask what clinical decisions the tool influences, such as "initiating a sepsis bundle, escalating level of care, activating rapid response."
The certification builds on the Joint Commission and Coalition for Health AI guidance of September 17, 2025, which says "Governance structures should include a designated individual(s) with appropriate technology expertise, ideally in AI if available, to lead implementation and use of AI tools across the healthcare organization." CHAI released its governance playbooks on May 27, 2026.
BEFORE
Hypothetical: a hospital runs an early warning score on its medical floors. Nursing leaders report too many low-value alerts overnight. The clinical informatics team proposes a new alert threshold and attaches a back-test on recent admissions, the expected change in alert volume and the plan for monitoring missed deterioration.
The hospital's AI policy says threshold changes need governance approval. The AI committee meets monthly, and the next meeting is three weeks away. No one has written authority to approve a threshold change between meetings.
The file is ready. The person who can release it is not.
WHAT THE WAIT COSTS
Night nurses keep working through alerts they have learned to tune out, which is its own safety risk. When the committee finally approves the change, the rationale lives in a slide presentation and a chat thread, which is hard to show a reviewer who asks how the decision was tracked.
AFTER
The change arrives as one decision packet: the current and proposed thresholds, the back-test, the risk categorization, the monitoring plan and the rollback trigger.
The designated AI lead, with delegated authority for threshold changes inside approved limits, signs off within two days with conditions. The approval memo is written at the moment of decision, the committee reviews it at its next meeting, and the record shows who decided, when and why.
HOW WISDOMTWIN FITS
WisdomTwin.ai is building a sovereign judgment layer for regulated enterprises. It is designed to give each named approver a role-specific judgment twin that prepares the decision packet the way that person reviews, with the evidence attached, so the decision moves faster and leaves an auditable record. It is designed to run on-premises, on-device or in your private cloud, and the named person still makes the call.
We don't replace judgment. We remove the wait.
TRY THIS THIS WEEK
Pick one clinical AI tool and list every decision about it that needs governance approval: go-live, thresholds, users and documentation.
For each decision, write down who can approve it between committee meetings.
Find the record of the last threshold change and check whether it would answer a reviewer's question about how the decision was tracked.
